Payment and Insurance

Payment is made at the end of each session by check or by cash. Credit cards cannot be processed.

Each session is 50 minutes and the fee is $120. This fee is also used for letter writing, consultation and professional phone calls.

If someone wants to use insurance for payment, a completed receipt is provided for the client to send to the insurance company for reimbursement to their home. A DSM diagnosis is required for all submitted claims.

Although most all insurance companies reimburse for therapy services listed, it is encouraged to first call the insurance company and provide the therapist’s name and full list of credentials (Jeff Dwarshuis LMSW, ACSW) to be assured of payment. Some managed care companies such as Priority Health or Blue Care Network will not reimburse unless they will do partial payment for an “off panel” provider. Also, a Blue Cross Blue Shield PIN number is available as well as an EIN number and NPI number. Some insurance companies will request this information when a claim for service is submitted. The therapist is responsible for providing this information.
